Job Title: Care Coordinator

West Side Family Health Care is seeking a highly skilled Care Coordinator to join our team.

Job Summary:

The Care Coordinator will work closely with the Care Coordinator Supervisor to develop and implement patient care plans, ensuring that patients receive comprehensive and coordinated care. This role requires strong communication and organizational skills, as well as the ability to work effectively with patients, families, and healthcare providers.

Responsibilities:

  • Develop and implement patient care plans in collaboration with the Care Coordinator Supervisor and healthcare providers.
  • Coordinate patient care services, including scheduling appointments, arranging transportation, and providing education and support.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
  • Monitor patient progress and make adjustments to care plans as needed.
  • Collaborate with other healthcare professionals to ensure comprehensive and coordinated care.

Requirements:

  • Bachelor's degree from an accredited university in a field such as Public Health, Public Administration, Business Administration, or Computer Science.
  • At least 2 years of experience working in healthcare or a related field.
  • Active license as a Medical Assistant or equivalent experience.
  • BLS/CPR certification.
  • Bilingual Spanish/English (preferred).
