Key Responsibilities- Promote Tribal Heritage and Services: Connect Ponca members with available services, resources, and programs to promote cultural heritage and well-being.
- Build Relationships and Trust: Establish strong relationships with Ponca members in assigned districts to promote services, build trust, and foster open communication.
- Identify Community Resources: Assist in identifying community resources that contribute to self-sufficiency and sustainable lifestyles.
- Provide Case Management and Support: Offer case management and document services offered, available, or provided to Ponca members.
- Maintain Accurate Records: Maintain accurate client and program files within approved software systems.
- Collaborate with Agencies: Cooperate and collaborate with Federal, State, local, and nonprofit agencies to provide comprehensive services.
- Report and Analyze Progress: Complete monthly reports outlining program goals, objectives, service plans, and achievements.
- Provide Direct Services and Advocacy: Provide direct services and advocate to meet identified needs of members.
- Assess Client Needs: Assess needs of Ponca members, including conducting intake activities, home visits, and attending meetings/hearings/appointments as deemed necessary.
- Partner with Child Welfare Program: Partner with the child welfare program to provide case management services and support to Ponca children and their families.
- Participate in Departmental Activities: Participate in departmental planning, staff meetings, committee assignments, certification coursework, and education events as directed.
- Facilitate Support Groups and Education: Facilitate support group and community education events as needed.
- Maintain Confidentiality: Practice strict confidentiality in accordance with all applicable Ponca Tribe of Nebraska policies.
- Meet Departmental Goals: Assist in meeting the goals and objectives of the Ponca Tribe of Nebraska.
- Education and Experience: Associate's degree in a human services-related field or equivalent combination of experience and education.
- Case Management Experience: Two years of experience in case management or advocacy.
- Community Resource Connection: Experience connecting with community resources.
- Interpersonal Skills: Interpersonal skills working with vulnerable populations.
- Computer Proficiency: Computer proficiency in Microsoft Word, Excel, and PowerPoint programs.
- Communication Skills: Excellent written and oral communication skills.
- Valid Driver's License: Valid driver's license and be insurable.
- Background Checks: Must successfully pass drug and alcohol testing, motor vehicle, education, character, and criminal background checks.
- Travel Requirements: Able and willing to travel in and out of the Ponca Service Delivery Area.
- Bachelor's Degree: Bachelor's degree in a human services-related field or equivalent combination of experience and education.
- Case Management Experience: Three years of case management experience.
- Department of Health and Human Services Experience: Experience working with Department of Health and Human Services for eligibility of TANF, SNAP, Medicaid/Medicare, disability, and/or Social Security.
- Budgeting Experience: Creating, managing, and facilitating budgeting activities.
- Native American Experience: One year experience working with Native Americans or Native American organizations.
- Public Law Indian Self-Determination Act Knowledge: Knowledge and understanding of Public Law Indian Self-Determination Act.
Preference will be given to Ponca Tribe of Nebraska members or members of a federally recognized tribe. However, all qualified individuals are eligible.
The Ponca Tribe of Nebraska is an at-will employer.
