Job Summary

Mission Hospital is seeking a skilled RN Care Manager to join our team. As a key member of our healthcare team, you will play a vital role in ensuring high-quality, patient-centered care by managing rehabilitative care to achieve optimum outcomes.

Responsibilities
  • Coordinate and direct care specific to the needs of each patient
  • Collaborate with the Rehab Program Director and Facility Case Management Director regarding departmental functions
  • Coordinate efforts within the Rehabilitation team
  • Ensure implementation of the patient's treatment plan that supports the patient's strengths, abilities, needs, and preferences
  • Facilitate the involvement of the patient throughout the rehabilitation process
  • Document the findings of the Discharge Planning Evaluation (DPE) and psychosocial assessments
  • Communicate the social, financial, or discharge needs and preferences of the patient/family/caregiver
Requirements
  • Minimum of 3 years in nursing practice
  • Clinical knowledge, IT skills, effective interpersonal communication, critical thinking, successful problem-solving, ability to work autonomously, innovative thinking, resourceful at navigating community resources, team building and leadership
  • Previous 5 years in nursing practice and 3 years care management experience preferred
  • Bachelor of Science in Nursing or bachelor's degree in a related field required
  • Current licensure as registered nurse with the North Carolina Board of Nursing
  • Nationally recognized certification in Case Management, such as ACM, CCM or ANA certification
