Position Overview:The Clinical Pharmacist plays a crucial role in ensuring the safe and effective use of medications, providing drug information, and monitoring drug therapy in accordance with established pharmacy policies and regulations.
Key Responsibilities:- Implement drug cost control measures.
- Manage polypharmacy and optimize medication management for patients with chronic conditions.
- Provide education and support to participants and staff regarding medication use.
- Conduct lab reviews, authorize prescriptions, and maintain accurate medication records.
- Assist in the training of new employees and ongoing education for staff.
- Act as a liaison between pharmacy services and healthcare teams to resolve distribution issues.
- Report incidents and adverse drug reactions as necessary.
Applicants should possess strong analytical skills, the ability to manage multiple priorities, and excellent customer service skills. A B.S. Pharm or Pharm D from an accredited institution is required, along with a valid NC Registered Pharmacist License.
Experience in a health-related field, particularly with geriatric populations, is preferred. Candidates must be capable of physical tasks associated with the role, including lifting and handling medications.
