As a Hospice Care Team Leader, you will play a pivotal role in the daily operations of our office, ensuring that our Inter-disciplinary Team functions smoothly while coordinating clinical efforts for our patients. Our Team Leaders serve as the central point of contact for both the office and clinical teams, offering support to families throughout the end-of-life journey and beyond. For many of our professionals, this role transcends a mere job; it is a profound calling.
Position Overview
The Hospice Care Team Leader is responsible for managing patient documentation, which includes creating and maintaining patient charts from admission through discharge. In this role, you will ensure that patient information flows seamlessly to all members of the Inter-Disciplinary Team (IDT) in a timely manner.
Key Responsibilities
- Document referral intake data and information for newly admitted patients.
- Ensure accuracy and completeness of all required documentation.
- Organize and file documents within patient charts and maintain proper records.
- Document any changes in the level of care appropriately.
- Compile re-certification and de-certification paperwork, notifying relevant providers of patient discharges.
- Facilitate communication among Inter-disciplinary Team members.
- Assist with IDT meetings, including agenda preparation and updates to care plans.
- Exceptional organizational skills are essential.
- Strong verbal communication skills, both in person and over the phone, with families, patients, and healthcare professionals.
- Flexibility to work various shifts, including evenings and weekends.
- Ability to maintain confidentiality of sensitive information.
- Proficiency in Microsoft Office applications.
A High School Diploma or General Education Degree with two to three years of relevant experience in healthcare; or an equivalent combination of education and experience.
