Job Summary

The Community Health Worker (CHW) will serve as a vital link between community members and healthcare providers, promoting education, outreach, and engagement to improve health outcomes. This role will focus on providing person-centered care, connecting individuals with resources, and fostering a supportive environment that welcomes and embraces diversity.

Key Responsibilities:

  • Represent Integral Care in the community, providing education and outreach about the CHW role and services.
  • Promote communication and understanding between community members and providers, increasing the use of health services and improving adherence to health recommendations.
  • Assist individuals in navigating and accessing resources that promote a healthy lifestyle, addressing social determinants of health.
  • Work with individuals to develop skills necessary to manage symptoms, obtain/maintain services, and access resources.
  • Provide recovery-focused, person-centered care with a trauma-informed perspective, promoting care continuity and addressing gaps in services.

Requirements:

  • Graduated from an accredited college or university with a Bachelor's degree in Human/Behavioral/Social Services or a related field.
  • Current and valid driver's license and a clean driving record.
  • Preferred: Prior/active military, bilingual, or experience working with underserved populations.
