PACT Clinician

The PACT Clinician provides psychosocial support as part of a multidisciplinary team to individuals with psychiatric experiences and mental illness. This role involves providing case management, counseling, therapy, and symptom management to clients in a community-based setting.
Key Responsibilities- Provide service coordination (case management) for an assigned group of clients, including coordinating and monitoring the activities of the individual treatment team (ITT).
- Develop, write, implement, evaluate, and revise overall treatment goals and plans in collaboration with the client and the ITT.
- Provide individual side-by-side support, counseling, therapy, and symptom management, ensuring immediate changes are made in the treatment plan as clients' needs change.
- Educate and support clients' families and advocate for clients' rights and preferences.
- Conduct holistic and comprehensive assessments along with other team members.
- Act as liaison and consult with community agencies and families to maintain coordination in the treatment process.
- Provide education and opportunity to other team members to engage in and learn about expressive therapies work (in-service training, staff wellness days, etc).
- Participate in on-call rotation for crisis intervention/support 7 days/week, 24 hours/day.
- Document client progress to maintain a permanent record of client activity according to established methods and procedures.
