Job Title: Advanced Practice ProviderJob Summary:

We are seeking a part-time Advanced Practice Provider to join our team at the York South Skilled Nursing and Rehabilitation Center in York, PA. This position offers a unique opportunity to work in a dynamic and growing sector of healthcare, with a focus on providing high-quality care to our residents.

Responsibilities:
  • Provide primary and urgent care services to nursing center residents in collaboration with other medical providers.
  • Work collaboratively with Center Leadership to achieve AlignMed Partners' mission of improving the lives we touch through the delivery of high-quality healthcare and everyday compassion.
  • Collaborate with the Medical Director and/or Attending Physician for patient-specific medical care.
Requirements:
  • Graduate from an accredited School of Nursing program, licensed as a registered nurse, and certified as a Nurse Practitioner.
  • Concentration and specialty track in Older Adult and/or Family Nurse Practitioner (FNP).
  • Minimum of two years' experience working with adult or geriatric populations.
  • APRN with an unrestricted professional license and DEA, for each state in which they practice.
