We are seeking a skilled Care Partner to join our Telemetry unit as a full-time care provider. This role is available for day shifts.
Job SummaryThis Care Partner position is responsible for providing direct care to assigned patients under the supervision of a Registered Nurse. The ideal candidate will be a Certified Nursing Assistant (CNA) with excellent communication and interpersonal skills.
Key Responsibilities- Take and record patient vitals, including temperature, blood pressure, and pulse
- Assist patients with daily tasks, such as bathing, dressing, and using the restroom
- Monitor patient progress and maintain accurate health records
- Foster strong relationships with patients and their families, ensuring a comfortable and supportive environment
- Aid communication between patients and healthcare teams, facilitating effective care
- Communicate with patients' families and provide updates on their loved ones' condition
- Help patients plan for hospital discharge and ensure a smooth transition to home or further care
- Certified Nursing Assistant (CNA) certification in the State of Tennessee or current enrollment in a professional nursing program
- Basic Life Support (BLS) certification
