Per Diem Advanced Practice Clinician

We are seeking an experienced Advanced Practice Clinician to join our team as a Per Diem Senior Community Care Provider. In this role, you will deliver high-quality care to our patients at home and in assisted living facilities, working closely with interdisciplinary teams to ensure seamless coordination of care.

About the Job

This position offers a flexible schedule with approximately 24 hours per month. You will be responsible for conducting comprehensive assessments, managing members through acute medical situations, and ensuring accurate documentation. Your ability to communicate effectively with interdisciplinary teams and provide patient education will be essential to your success. As a member of our team, you will receive competitive compensation and benefits, including an estimated salary of $85,000 - $120,000 per year, depending on your experience and qualifications.

Requirements

To be considered for this role, you must have an active and unrestricted license in the state of Texas, a graduate degree from an accredited Master of Science Nursing or Doctor of Nursing Practice program, and at least one year of clinical experience. Additionally, you should be able to obtain post-hire DEA licensure/prescriptive authority and access to reliable transportation.
