About the RoleAs a Weekend Home Health Nurse, you will provide skilled nursing care to patients in their homes, working closely with our interdisciplinary team to develop and implement individualized care plans. You will assess patients' physical and emotional needs, provide education and support to patients and their families, and collaborate with other healthcare professionals to ensure seamless care transitions.
Responsibilities- Provide skilled nursing care to patients in their homes, including assessments, treatments, and education
- Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams
- Collaborate with other healthcare professionals to ensure seamless care transitions and coordination
- Provide education and support to patients and their families on disease management, medication adherence, and self-care
- Document patient progress and outcomes in accordance with regulatory requirements
- Current and unrestricted RN licensure
- Minimum of one year of experience in home health care or a related field
- Strong assessment and communication skills
- Ability to work independently and as part of a team
- Flexibility to work a variety of shifts, including weekends and holidays
