Key Responsibilities- Provide medication therapy management services to patients with complex medical conditions, including optimization of drug therapy, identification and resolution of drug-related problems, and improvement of therapeutic outcomes.
- Collaborate with healthcare providers to develop and implement individualized medication plans that meet the unique needs of each patient.
- Conduct medication reviews and provide recommendations for medication therapy optimization, including dose adjustments and laboratory monitoring.
- Develop and implement education programs for patients, families, and healthcare providers on medication management and adherence.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- Eligibility for licensure to practice pharmacy in the Commonwealth of Massachusetts.
- Completion of 20 continuing education credits per year to maintain licensure.
- Ability to work collaboratively as a member of the patient care team.
- Strong communication and interpersonal skills.
- Ability to prioritize and organize work effectively.
This is a full-time position with rotating shifts, including weekends and holidays. The successful candidate will be required to work in a fast-paced environment with frequent interruptions and deadlines.
