The Medical Care Specialist - Geriatric Services at Abode Care Partners provides high-quality, personalized medical care to seniors and patients with complex needs. This role involves conducting thorough medical evaluations, collaborating with healthcare professionals, and maintaining accurate patient medical records.

Responsibilities:
  • Conduct evidence-based medical evaluations, developing comprehensive plans of care for patients.
  • Collaborate with caregivers and other healthcare professionals to ensure seamless, interdisciplinary care.
  • Maintain accurate, up-to-date patient medical records.
Qualifications:
  • Active state Nurse Practitioners license.
  • Current Nurse Practitioner National Board Certification.
  • Active state DEA licensure.
  • Master's degree in Nursing (MSN).
  • Five or more years of nursing experience; at least two years of nurse practitioner experience preferred.
  • Strong interpersonal communication skills – verbal and written.
  • Excellent time management skills with accountability for practitioner's daily workflow.
