We are seeking a highly skilled Registered Nurse Case Manager to join our team at the University of Miami. As a key member of our healthcare team, you will play a critical role in coordinating patient care and ensuring the best possible outcomes for our patients.
Key Responsibilities- Coordinate patient care from admission to discharge, ensuring seamless transitions and effective communication with healthcare teams.
- Monitor patient progress, identify potential issues, and develop and implement plans to address them.
- Collaborate with medical teams to facilitate patient movement through the system and ensure timely and effective care.
- Provide education and support to patients and families, empowering them to take an active role in their care.
- Serve as a liaison between patients, families, and healthcare personnel, ensuring that necessary care is provided promptly and effectively.
- Promote quality care, ensuring that patients receive medically appropriate services in a timely and cost-effective manner.
- Facilitate ongoing insurance authorization and regulatory notifications, ensuring compliance with policies and procedures.
- Maintain knowledge of insurance reimbursement policies and stay up-to-date on industry developments.
- Relies on experience and judgment to plan and facilitate discharge and transition plans, ensuring they meet the physical, social, and emotional needs of patients.
- Graduate from an accredited school of nursing, Bachelor's degree (BSN).
- Valid Florida Registered Nurse License, BLS certification from the American Heart Association required.
- Minimum 2 years of relevant experience, with a minimum of 1 year in Hospital Case Management/nursing.
- Working knowledge of patient assessment and medical terminology.
- Ability to communicate effectively in both oral and written form.
- Ability to recognize, analyze, and solve a variety of problems.
- Ability to analyze, organize, and prioritize work under pressure while meeting deadlines.
- Ability to maintain effective interpersonal relationships.
