Patient Care Coordinator

We are seeking a dedicated and compassionate Patient Care Assistant to join our team at Bon Secours Mercy Health. As a key member of our healthcare team, you will provide direct patient care under the guidance of the Registered Nurse or designated health care professional.
Key Responsibilities- Provide basic patient care, including care and comfort, record vital signs, personal care and hygiene, and mobility, including unit-based specialty duties.
- Act as liaison between patient and nurse to report changes or concerns to nurse.
- Provide high-level customer service to all patients, patient's family, visitors, and all employees.
- Accurately and precisely document patient care when applicable.
- Utilize professional, verbal, and written communication skills at all times.
- Preference for recent experience in acute care and/or long-term care facility.
- Ability to clearly communicate with other members of the healthcare team.
- Ability to understand and follow directions of healthcare team.
- Knowledge of patient and healthcare safety standards and regulations.
- Basic Life Support certification (American Heart Association preferred).
- Nursing Student with First Clinical Rotation complete or state testing complete, or 1 year of experience in a clinical setting.
- High School/GED (required).
