About the Role

We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team at Amedisys, a leading provider of home health, hospice, and high-acuity care. As a RN, you will play a vital role in delivering patient-centered care and support to individuals and their families in the comfort of their own homes.

Key Responsibilities
  • Perform initial nursing assessments on new admissions to identify patient needs and develop individualized care plans.
  • Collaborate with healthcare teams to ensure continuity of patient care and make referrals to other hospice care disciplines as needed.
  • Communicate patient progress to clinical managers, physicians, and care teams to ensure seamless care coordination.
  • Develop, prepare, and maintain accurate and timely patient care progress records in accordance with care center policies.
  • Provide education and support to families on patient condition, needs, and care.
  • Perform on-call responsibilities and provide services to patients and families as assigned.
Requirements
  • Current RN license specific to the state(s) assigned to work.
  • One+ year of nursing experience required.
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and liability insurance.
