The mission of our Geriatric and Extended Care programs is to alleviate the challenges faced by older, frail, and chronically ill patients, as well as their families and caregivers. We aim to enhance each patient's functional independence through tailored services that adapt to their evolving health care needs.

Our offerings include Community Residential Care, VA Community Living Centers, Community Nursing Homes, and State Veteran Homes, all designed to support veterans who require assistance in their daily lives. We also emphasize research, education, and clinical advancements through our Geriatric Research Education and Clinical Centers.

Our interdisciplinary services, including Geriatric Evaluation & Management, Geriatric Primary Care, and Inpatient Respite, cater to the diverse needs of elderly patients. Additionally, our Hospice and Palliative Care services provide comprehensive support for individuals with advanced illnesses.
