We are seeking a highly skilled Registered Nurse to join our team at Woodmont Health Care Center in Fredericksburg, VA. As a Care Management Registered Nurse, you will play a vital role in providing exceptional patient care and collaborating with our interdisciplinary team to develop effective care plans.
Key Responsibilities:- Conduct comprehensive nursing assessments and work collaboratively with the nursing team, other disciplines, patients, and families to develop and implement individualized care plans.
- Establish realistic, measurable short- and long-term patient goals and develop interventions to minimize rehospitalizations and optimize patient outcomes.
- Delegate patient care responsibilities to staff and supervise them to ensure that tasks and interventions are implemented effectively.
- Communicate patient information with assigned staff and between shifts to ensure seamless care delivery.
- Graduate of an approved school of nursing and currently licensed by the State Board of Nursing.
- CPR Certification is required.
- Competitive hourly rate: $35 - $38 per hour.
- Full-time position with a dynamic and supportive work environment.
