Psychiatric Rehabilitation Specialist

We are seeking a highly skilled and compassionate Psychiatric Rehabilitation Mobile Worker to join our team at Community Guidance Center. As a key member of our mental health services team, you will provide site-based or mobile psychiatric rehabilitation services to consumers, utilizing your knowledge of psychiatric rehabilitation philosophy, PA State guidelines, and Boston University approach.
Key Responsibilities- Develop and implement individual rehabilitation plans (IRPs) to support consumers in achieving their goals.
- Assess individuals to evaluate their readiness for psychiatric rehabilitation and provide coaching and support as needed.
- Provide group or individual instruction based on the IRP, utilizing social learning and recovery methods.
- Facilitate consumer contact with family members, school personnel, social service agencies, and other service providers.
- Organize and facilitate meetings with family members and other service providers to ensure continuity of care.
- Make appropriate referrals and facilitate consumers' transition to other services in preparation for discharge.
- Maintain accurate and complete documentation and fiscal standards.
- Comply with quality assurance guidelines and maintain awareness of all Federal, State, and Managed Care laws, policies, and procedures.
- Associate's or Bachelor's degree in a related field.
- At least 1 year of work experience in mental health direct service or a CPS certificate with 1 additional year of paid or volunteer work experience in mental health direct service.
- High school diploma or GED with 2 years of work experience in human services, including 1 year of mental health direct service.
- Act 33, 34, FBI Clearances, and Act 31 (Mandated Reporter Training) certification.
- CPRP certification or obtain within 2 years from the date of hire.
- Training and experience in facilitation of groups.
- Ability to maintain control of consumers in large group settings and demonstrate emotional intelligence.
