Salary Information

The estimated salary range for this position is $120,000 - $180,000 per year, depending on experience and qualifications. In addition to competitive salaries, we offer comprehensive benefits packages, including health insurance, 401k plans, tuition reimbursement, and paid time off.


Job Description

We are seeking a skilled Hospice Nurse Practitioner to join our team. As a key member of our interdisciplinary group (IDG), you will perform Face-to-Face (F2F) encounters and gather clinical information to support patient eligibility. Your findings will be communicated to our attending physicians for certification or recertification.


Responsibilities
  • Conduct thorough physical assessments within regulatory time frames established by CMS.
  • Review medical treatment plans, diagnostic tests, lab results, and medication lists to inform your recommendations to the IDG.
  • Notify us when unable to complete F2F visits according to schedule.

Requirements
  • A Master's or higher degree in Nursing.
  • Graduate-level nurse practitioner education program accreditation by a national accrediting body.
  • A current state license as a registered nurse and advanced practice nurse.
  • National certification as a Nurse Practitioner (ANCC or AANP).
  • A collaborative agreement in place.
