ESSENTIAL QUALIFICATIONS
Must hold a current RN license issued by the State Board of Nursing.
A minimum of one year of clinical experience is preferred, particularly in home health, medical, surgical, or critical care settings.
Demonstrates effective communication skills tailored to the diverse age groups of patients served. Adheres to established professional standards and ethical principles.
Maintains good physical stamina and mental well-being. Completion of the agency's pre-employment health assessment is required.

Must possess a valid driver's license and have access to a reliable, insured vehicle that meets state and/or agency standards.

Flexibility and cooperation in fulfilling job responsibilities are essential.
Exhibits strong observational skills, sound clinical judgment, and effective oral and written communication abilities.
Self-motivated with the capacity to work independently; possesses strong organizational skills.

Must hold a current and unrestricted license to practice as a Registered Nurse in the state of assignment.

A minimum of one (1) year of clinical nursing experience is required.
To fulfill CMS requirements for psychiatric nursing in a home care context, a registered nurse must meet one of the following criteria:

A Master's degree in psychiatric or mental health nursing.

A Bachelor's degree in nursing accompanied by one year of recent nursing experience in an active treatment unit within a psychiatric or mental health facility or outpatient clinic.

A diploma or associate's degree with two years of recent nursing experience in an active treatment unit within a psychiatric or mental health facility or outpatient clinic.


OVERVIEW OF JOB RESPONSIBILITIES

As a psychiatric registered nurse specializing in behavioral health within home health, you will have the opportunity to deliver personalized psychiatric and skilled nursing care to patients in their residences.

Your primary responsibilities will involve collaborating with mental health professionals to develop a home care plan that supports individuals and their caregivers dealing with conditions such as dementia, Alzheimer's, schizophrenia, anxiety, grief, depression, and other psychiatric disorders in a home environment.



KEY RESPONSIBILITIES
Develops, prepares, and maintains personalized patient/client care documentation in alignment with the care plan. Ensures accurate documentation is submitted within one business day of a visit or phone call.
Assesses and provides education and information to patients/clients and their families/caregivers that is relevant to their diagnosis and care plan.

Engages in the coordination of home health services, consistently reporting the patient's/client's condition, status, and need for additional services (HHA, OT, PT, MSW, ST, Dietitian) to the RN Case Manager and/or Branch Supervisor, or documents the rationale for not doing so.

Utilizes equipment and supplies efficiently and effectively.
Provides nursing services as required, in accordance with the care plan signed by the physician.

Demonstrates proficiency in all required skills for the agency, which may include but are not limited to venipunctures, infusion therapy, wound care, and aseptic techniques.

Continuously assesses the client's care needs. Documentation addresses issues identified in ongoing assessments or provides justification for not doing so.

Supervises home health aides at least every 14 days, instructing them in client care duties as necessary, and assesses the client's need for unskilled care, advising the case manager of any necessary revisions to the care plan.

In ACHC Accredited offices, supervises LVN/LPN every 55-60 days, with or without the LVN/LPN present in the home. Oversees the care plan delivered by LVN/LPN during recertification/discharge visits and provides necessary training.
Responsible for adhering to all agency policies and procedures regarding service delivery, documentation, and care coordination.
Participates in on-call rotation as assigned by the supervisor.
Engages in the agency's Quality Improvement program and can articulate personal responsibilities and activities.
Performs duties within the scope of training and experience.

Participates in community outreach and public relations activities that enhance the agency's role in the healthcare delivery system.

Seeks and gathers sufficient information to function effectively in the assigned healthcare setting. Remains current in nursing trends and knowledge. Participates in in-service training programs.
Administers medications and/or treatments as prescribed by the physician.

Initiates and implements appropriate preventative, therapeutic, and rehabilitative procedures and maintains communication regarding patient/client care with the physician and other involved disciplines.

Accepts responsibility for performing specialized procedures only when qualified.

Observes patients/clients for changes in condition, discusses these changes with the patient/client or family/caregiver, and reports necessary changes to the attending physician and/or RN Case Manager.

Documentation reflects communication and the condition of the patient.

Educates, supervises, and counsels patients/clients and their families/caregivers on home nursing procedures, self-care techniques, and other care needs as appropriate to the patient's/client's condition.

Provides medication, dietary, and other instructions as ordered by the physician and identifies opportunities for health counseling with patients/clients and their families/caregivers.

Assists patients/clients in maintaining a safe and optimal environment.

Evaluates and assesses the medical, cognitive, emotional, psychosocial, safety, developmental status, and behavioral health/psychiatric nursing needs of the patient to determine the appropriateness of care and criteria for acceptance into behavioral health/psychiatric home health care.

Evaluates the patient's adherence to the medical and psychiatric care plan and communicates closely with the physician, interdisciplinary team, agencies, and institutions to ensure continuity of care regarding the patient's progress.

Acts as a case manager in coordinating care for behavioral health/psychiatric patients.

Regularly reviews treatment plans under the guidance of a physician or psychiatrist, which includes nursing actions and interventions, goals, and discharge summaries.

Administers IM medications, performs lab draws and specimen collections, and educates patients and caregivers on appropriate medical techniques. Performs other nursing treatments in accordance with the care plan.

Must fulfill the required continuing education hours for state certification as a nurse in Texas, as applicable.

Additionally, agency personnel are expected to take personal responsibility for other educational activities to enhance job-related skills and competencies.

All agency personnel must attend mandatory educational programs.
