Job Summary

We are seeking a compassionate and skilled Hospice Chaplain to join our team at All County Home Health and Hospice. The successful candidate will provide spiritual care services to patients and their families, ensuring a peaceful and dignified end-of-life experience.

Key Responsibilities
  • Conduct spiritual assessments of patients and families to identify their unique needs and preferences.
  • Develop and implement individualized plans of care that address the spiritual, emotional, and social needs of patients and their families.
  • Provide direct spiritual care to patients and their families, including counseling, support, and guidance.
  • Collaborate with the interdisciplinary team to ensure comprehensive care and support for patients and their families.
  • Maintain accurate and timely records of spiritual care services provided.
Requirements
  • Master's degree in divinity or a related field.
  • Experience working in hospice care or a related field.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Comfort working in an interfaith setting.
