JOB SUMMARY:
The role of the Senior Care Coordinator is pivotal in ensuring that residents receive the highest standard of care and support in our assisted living and memory care communities. This position is not just a job; it is a chance to make a meaningful impact in the lives of our residents.

KEY RESPONSIBILITIES:
  • Foster significant relationships with a designated group of seniors and their families, assisting them with daily living activities while respecting their individual care preferences.
  • Monitor and report any changes in residents' health conditions to management, suggesting necessary adjustments in care plans.
  • Support residents in developing life skills and engaging in enriching activities tailored to their personal profiles.
  • Integrate a variety of sensory experiences into the daily routines of residents.
  • Collaborate in the creation and regular updates of Individualized Service Plans (ISP) for residents.
  • Maintain accountability for the well-being of assigned residents throughout the shift, ensuring their safety and comfort.
  • Work alongside the community team to uphold compliance with safety regulations and promote risk management initiatives.
  • Attend to the dining needs of residents and guests, ensuring a high standard of service and hospitality.
  • Encourage and assist residents in participating in life skills and activities as outlined in their ISP.
  • Uphold our commitment to service excellence through our guiding principles.

QUALIFICATIONS:
  • A strong commitment to serving seniors with outstanding customer service skills.
  • A positive demeanor, adaptability to perform various roles in service to residents, and the ability to thrive in a collaborative environment are essential for success.
  • A high school diploma or GED is required, as per state regulations.
  • In applicable states, maintaining relevant certifications is necessary.
  • Candidates must be at least 18 years old.
  • Previous experience in senior care is preferred.
  • Ability to make informed decisions that prioritize the residents' best interests.
  • Proficient in both written and verbal communication skills.
  • Strong organizational and time management abilities.
  • Demonstrated capacity for sound judgment, problem-solving, and decision-making.
