Home Healthcare Nurse Manager - Case Manager Position
Work Schedule: Wednesday - Friday, 8:30 AM to 5:00 PM
BrightStar Care is seeking a dedicated and motivated Registered Nurse (RN) to join our team as a Nurse Manager in the Portland area. The ideal candidate will have experience in home care agencies and a passion for delivering high-quality care. Key Responsibilities:
- Communicate effectively with patients, families, and case managers.
- Conduct home care assessments and develop care plans in collaboration with physicians and patient needs.
- Perform supervisory home visits to oversee cases and staff.
- Conduct competency skills checks for caregivers and nurses in our clinical lab.
- Assist with training, development, and ongoing education programs.
- Ensure compliance with Joint Commission standards and State/Federal regulations.
- Occasionally provide direct care to clients and offer training to field staff both in-home and on-site.
Requirements:
- Current, unencumbered Registered Nurse license to practice in Oregon.
- Preferred: Bachelor's degree in Nursing and graduation from a National League of Nursing accredited program.
- Minimum of three (3) years of clinical experience, with at least half of that time in home health or a related healthcare organization.
- Case management skills required.
- Experience with adult and senior care.
- Proven ability to supervise and direct staff.
- Strong nursing assessment skills.
- Adherence to professional standards of practice.
- Excellent interpersonal and customer service skills.
- Ability to treat clients, staff, and the public with respect, courtesy, and professionalism.
- Policy development experience is a plus.
- Knowledge of and experience with Joint Commission standards is desirable.
