Nurse Coordinator

Plains Area Mental Health, Inc. is a Community Mental Health Center dedicated to providing high-quality mental health services to the communities we serve. Our mission is to deliver compassionate and respectful care, prioritizing each individual's dignity, worth, and potential for growth.
We are seeking a full-time Nurse Coordinator to join our team in various locations across Northwest Iowa. This role requires travel to our satellite offices as needed. The position involves working 40 hours per week, Monday through Friday.
Compensation and Benefits
- Competitive salary range: $26.27 to $31.52 per hour, depending on experience

Job Responsibilities
The Nurse Coordinator will be responsible for:
- Coordinating care with Primary Care Physicians, Pediatricians, Psychiatrists, and other medical or mental health treatment providers
- Performing whole-person assessments and making recommendations to the internal Care Teams on interventions
- Providing education and awareness for preventive care and health promotion
- Promoting smoking prevention and cessation, substance abuse prevention and cessation, nutritional counseling, obesity reduction, and increased physical activity
- Engaging members and/or caregivers on alternatives to emergency or hospital care
- Participating in hospital discharge processes
- Performing medication reconciliation and aiding with medication, treatment management, and adherence, including administering oral and IM medications as needed
Requirements
- RN or BSN License in Iowa
- Valid Driver's License and Insurance
Preferred Qualifications
Bilingual skills are a plus, but not required.
