Center for Elders' Independence is seeking a skilled Physical Therapist to join our interdisciplinary team as a Per Diem Float Physical Therapist. This is a non-benefitted position that requires flexibility and adaptability to work at various centers across the Bay Area.
Key Responsibilities- Collaborate with the primary care provider team and the interdisciplinary team in evaluating agency participants by applying muscle, nerve, joint, and functional ability tests.
- Develop and implement individualized plans of care to maximize participants' independence and safety.
- Instruct participants and caregivers in the use of mobility-related devices, such as wheelchairs, braces, crutches, canes, and prosthetic and orthotic devices.
- Work with vendors to obtain rehabilitative equipment and supplies for participants.
- Provide in-service training to other members of the health team.
- Attend interdisciplinary team conferences and actively participate in the development of the plan of care.
- Evaluate participants' home environments and make recommendations as needed.
- Document thoroughly, accurately, and timely, and in accordance with Physical Therapy practice standards and CEI policy.
- Maintain a safe working environment and follow CEI's Safety Policies and Procedures.
- Possession of a minimum of a Bachelor's degree and graduation from a program of Physical Therapy approved by the Commission on Accreditation in Physical Therapy Education.
- Current licensure as a Physical Therapist by the Physical Therapy Board of California.
- Preferred, one (1) year of experience working as a Licensed Physical Therapist with the frail elderly population.
- Current CPR and First Aid certification on file.
- A valid California driver's license, current vehicle insurance, and access to a reliable vehicle.
- Ability to travel to multiple work sites and participants' residences.
Center for Elders' Independence is a PACE (Program of All-Inclusive Care for the Elderly) organization that uses an interdisciplinary team approach to care planning and care implementation for the purpose of providing high-quality, affordable, integrated health care services to the elderly.
