Licensed Practical Nurse

As a Licensed Practical Nurse - Clinic II at Kaiser Permanente, you will provide direct and indirect nursing care in an outpatient setting under the direction of a Registered Nurse and medical orders from a Practitioner. Your responsibilities will include reviewing and preparing medical records, initiating follow-up for medical records, and identifying and preparing orders for patient needs.
Key Responsibilities- Implement basic nursing procedures per plan of care, administer treatments, immunizations, and medications as prescribed by the Practitioner, and prepare medications for Practitioners to use.
- Provide laboratory, x-ray, and other reports to the Practitioner for review, notify patients of normal results as directed by the Practitioner, and consider special needs of different age groups when providing care.
- Document implementation of orders/interventions, record findings, instructions, and patient responses on appropriate forms or online records using department and/or JCAHO standards.
- Collaborate with RN/Practitioner to revise care plan when needed, demonstrate and apply competency in basic nursing skills and use of equipment, and maintain current knowledge and skills needed to perform position.
- Perform efficiently in emergency patient care/clinic situation, follow established procedures, and inform appropriate Practitioner/RN.
- Assign basic nursing tasks to unlicensed personnel as appropriate and according to department/unit standards, provide clear directions, and act as a resource person to unlicensed staff.
- Follow discharge plan developed by Practitioner or RN, process and/or follow-up with referrals as directed by Practitioners, and provide instruction and pre-printed information to patients as directed by RN and/or Practitioners.
- Manage all patient vaccine data, including updating Hawaii Immunization Registry System (HIR), Electronic Health Records (I), and all patient and/or external records at each patient visit.
- Licensed Practical Nurse (Hawaii) required at hire
- Basic Life Support from American Heart Association
- Demonstrated knowledge of and skill in conflict resolution, customer service, interpersonal relations, oral communication, problem solving, teamwork, and written communication
- Talking to co-workers, customers, outside vendors, and on the telephone
- Reading, writing, speaking, and understanding English
- Training/giving and receiving instructions
- Mathematical ability, attention to detail (e.g., organization, prioritization, proofing), concentration, and alertness
- Previous nursing experience
- Previous clinic experience
- Demonstrated knowledge of and skill in word processing and spreadsheet PC applications
