Patient Care Advocate

About the Role:
As a key member of our team at Serenity Community Care, the Patient Care Advocate plays a vital role in ensuring seamless care coordination for our patients. This part-time position involves providing consultative and advisory support to patients, families, and healthcare professionals to address barriers to access and care.
Duties and Responsibilities:
- Conduct comprehensive assessments of medical, behavioral health, and social service needs for assigned enrollees
- Provide disease-specific education and information on community resources
- Collaborate with community providers and resources to obtain needed services and support
- Request and coordinate team and patient meetings as needed or requested by patient/family and/or team
Requirements:
- BA degree and minimum 2 years of relevant experience in Human Care Services
- Excellent communication and team skills, including ability to form strong collaborative interdisciplinary partnerships across care settings
- Sound computer knowledge and skills, including aptitude for using health information technology to guide activities
Salary and Benefits:
- $20-$28 per hour (estimated)
- Part-time contract position with a minimum of 15 hours per week
