Job Requirements
We are seeking an experienced Registered Nurse to join our team as a Clinical Care Coordinator. In this role, you will be responsible for delivering high-quality patient care and coordinating healthcare services.

About the Position
This is a full-time position that offers a competitive salary range of $62,000 - $82,000 per year. You will work Monday to Friday, with no on-call duties. However, you may be required to participate in weekend and evening rotations in the future. The office is located at Harbor Gardens, 1650 Metropolitan Street, 15233.

About the Role
You will be responsible for providing direct patient care, evaluating outcomes, and adjusting the nursing care process as needed. This will involve conducting comprehensive face-to-face assessments, reviewing medical records, and identifying gaps in care. You will also develop individualized plans of care in collaboration with primary care providers and interdisciplinary teams. These plans will focus on promoting symptom management, healthy lifestyles, and reducing unnecessary emergency room utilization and hospital admissions.

Your Key Responsibilities
* Deliver high-quality patient care and coordinate healthcare services
* Conduct comprehensive face-to-face assessments and identify gaps in care
* Develop individualized plans of care in collaboration with primary care providers and interdisciplinary teams
* Collaborate with patients, families, and healthcare providers to promote patient-centered care
* Participate in quality improvement initiatives to enhance patient care
