Job Summary

We are seeking a skilled LPN to provide individual medical support and education to individuals in assigned programs. The successful candidate will advocate for all medical needs of assigned program participants, focusing on optimal health for all individuals.

Key Responsibilities
  • Assess and coordinate healthcare services for participants, instituting appropriate follow-up and communicating all medical issues to relevant staff members.
  • Assist and transport individuals in designated programs with their medical appointments, as recommended by their physicians and DDS standards.
  • Document all nursing interventions and changes affecting the consumer in appropriate areas, including TIER, progress notes, and staff communication logs.
  • Review ISPs for current knowledge of programming in self-medication, behavior plans, hygiene skills, and physical therapy plans.
  • Maintain current medication and treatment orders, noting any side effects or adverse effects and reporting to relevant staff members.
  • Maintain health records according to DDS standards, ensuring that necessary documentation is up to date and communicated to staff.
  • Monitor for the observance of safety protocols and meet with the Health Care Director or program residential RN for supervision and direction as needed.
Qualifications
  • LPN or RN with current MA license
  • One year working with individuals with MR/DD
  • Valid Driver's License required
