The Staff Pad partners with a non-profit healthcare system in Helena, Montana, to deliver exceptional care and serve the local community.

Salary Information

$60,000 - $80,000 per year based on experience and qualifications.

Job Responsibilities

The RN Care Manager plays a vital role in facilitating efficient and cost-effective patient progress through the Emergency Room and acute episode of care. This involves timely discharge planning, advocating for patients, and identifying potential financial barriers to care.

  • Collaborate with medical staff to confirm admission status, treatment goals, and clinical milestones.
  • Meet with patients and families to assess navigation and transition needs, identifying potential financial barriers and making referrals as necessary.
  • Utilize pre-determined high-risk screens to identify patients with transition challenges, assigning them to Social Worker Counselors for focused management.
  • Verify admission diagnosis and correct admission status, promoting adherence to clinical protocols and evidence-based interventions.
  • Provide point-of-care coaching to medical staff on documentation improvement, safety, and quality indicators.
  • Initiate referrals for home health, hospice, DME, rehab, and other services, aligning patient needs with placement options.
  • Demonstrate proactive communication to influence treatment plans and progression of care, advocating for patients and organizational stakeholders.
Performance Expectations
  • Function as a member of a self-directed Care Management work team, ensuring appropriate coverage based on departmental needs and population served.
  • Evaluate patients' status and need for acute care services using InterQual severity of illness/intensity of service criteria.
  • Work closely with ED physicians and staff to ensure patients not admitted receive appropriate outpatient referrals and follow-up.
  • Participate in performance improvement activities and prepare monthly updates on work accomplishments.
  • Collaborate with all personnel within the department and throughout St Peter's Hospital.
Requirements
  • Minimum 3-5 years of acute care experience.
  • Able to use electronic software applications related to case management activities.
  • Ability to rotate with other CM/UR RNs for weekend and holiday coverage.
