Job Summary: We seek a qualified Family Medicine Physician to provide medical care to our patients in accordance with the best current standards of professional practice. The ideal candidate will be responsible for obtaining histories, performing physical examinations, prescribing treatment and medication, and providing relevant information about our Patient Centered Medical Home and Chronic Care Management programs.

Key Responsibilities:

  • Conduct thorough patient assessments, including taking histories and performing physical examinations.
  • Prescribe treatment and medication for conditions within the scope of family medicine, following evidence-based medicine principles.
  • Provide patients with information about our Patient Centered Medical Home and Chronic Care Management programs, ensuring they understand the benefits and services available to them.
  • Collaborate with other healthcare professionals to deliver comprehensive patient care, including consulting, referring, and collaborating with specialists as needed.
  • Document all patient interactions and care provided in the medical record, adhering to established standards and regulations.
  • Participate in ongoing educational programs to maintain clinical skills and stay up-to-date with the latest medical research and guidelines.
  • Ensure patient safety and well-being by following established protocols and taking prompt action on abnormal test results or physical findings.
  • Contribute to the development and implementation of programs to enhance patient care and satisfaction, including preventive care and diagnostic screening initiatives.
  • Work collaboratively with peers to ensure seamless coverage and provide high-quality care to our patients.

Requirements:

  • Medical Doctoral Degree (MD/DO) from an accredited institution.
  • Current licensure and registration with the New York State Department of Education.
  • Basic Life Support certification.

Work Environment: This position requires regular interaction with patients, colleagues, and other healthcare professionals. The work environment is typically moderate, with occasional exposure to human blood or other potentially infectious materials.
