We are currently seeking Registered Nurses to conduct Initial Patient Assessments and Subsequent Follow-Up Visits.

The RN will evaluate patient requirements, formulate the Care Plan, and perform follow-up visits as necessary. For visits related to Physical Therapy, Occupational Therapy, or Speech-Language Pathology, follow-up is not required.

Qualifications:

  • Minimum of 2 years of clinical RN experience
  • Preferred experience in home healthcare
  • Familiarity with AlayaCare is a plus
  • Experience with Start of Care processes is advantageous
Requirements:
  • Valid NYS Registered Nursing License
  • Current BLS certification
  • Physical examination within the last year
  • QuantiFERON test within the last year
  • MMR Titers documentation
  • NPI number
  • Malpractice insurance coverage
  • Two forms of government-issued identification
  • Two professional letters of recommendation
