Key Responsibilities

  • Provide one-on-one care and skilled services to patients in their own homes, including admissions for all levels of patient care.
  • Collaborate with patients and caregivers to educate and empower them to manage their conditions and remain in their own homes.
  • Conduct in-home evaluations, assessments, and care planning to ensure patients receive the necessary services to meet their needs.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
  • Work collaboratively with the interdisciplinary team to develop and implement individualized care plans.

Requirements

  • Education
    • Graduate of an approved nursing program.
  • Experience
    • Minimum one (1) year experience as a professional nurse within the last three (3) years.
  • Skills
    • Excellent communication and interpersonal skills.
    • Ability to work independently and as part of a team.
    • High level of organization, responsibility, and self-management.
    • Ability to provide reliable transportation as required by the nature of the position.
  • Licensure/Certification/Registration
    • MO RN License.
    • BLS certification within 90 days of hire.
    • Active Driver's License.
