Job Summary

The State of New Mexico is seeking a highly skilled and compassionate Ombudsman Care Transition Specialist to join our team. This role is responsible for providing person-centered care transition services to older adults and adults with disabilities, ensuring a safe and successful transition from institutional care to community-based settings.

Key Responsibilities
  • Conduct thorough assessments and develop personalized care plans to address the unique needs of each client.
  • Provide education and advocacy to clients and their families, empowering them to make informed decisions about their care.
  • Collaborate with healthcare providers, social services, and community organizations to ensure seamless care transitions.
  • Investigate and resolve complaints related to care provider services, advocating for the rights and dignity of clients.
  • Represent the interests of clients before government agencies, seeking administrative and legal remedies to protect their well-being.
Requirements
  • Bachelor's degree in social work, criminal justice, psychology, or a related field.
  • At least one year of experience working with aging and disability clients, with a focus on person-centered approaches.
  • Knowledge of Medicaid, Medicare, and Social Security benefits, as well as long-term care services and supports.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
Working Conditions

This role requires a valid driver's license, reliable transportation, and the ability to work in a fast-paced office environment. Travel is required, and the incumbent must be comfortable working in stressful situations and meeting deadlines.
