Job Title: Driver

Community Wellness Partners is seeking a skilled and safety-conscious Driver to join our team. As a Driver, you will be responsible for the safe transportation of residents in our Adult Day Health Care program, Social Adult Day Care, Active Day, Non-Emergency Medical Transports, Dialysis Transports, Laundry Transports, Activities trips, and special transportation requests approved by the Transportation Team Leader or Manager.

Key Responsibilities:
  • Operate facility transportation vehicles, including cars, trucks, and multi-passenger vans, with a focus on safety.
  • Report to the Transportation Team Leader and maintain defensive driving at all times, responding to safety signs, traffic, and road conditions.
  • Assist registrants, residents, family members, and other passengers in boarding and exiting the vehicle, ensuring safe destination and arrival.
  • Interpret driving directions to plan the most efficient route and follow specific regularly scheduled routes for Adult Day Health Care/Social Adult Day Care/Doctor Appointments.
  • Provide transportation for special functions outside the facility and transport residents back and forth to appointments as requested.
  • Fuel vehicles as necessary or assigned and clean both the exterior and interior of fleet vehicles as needed or assigned.
  • Pick up and/or drop off vehicles to and from our repair shops as assigned.
  • Respond immediately to accident or medical emergencies with proper notification to designated parties relative to the accident/incident.
  • Maintain accurate, up-to-date records as required.
  • Notify the Transportation Team Leader of any vehicle problems for immediate attention to ensure vehicle safety at all times.
  • Must possess a valid/clean New York state driver's license in good standing meeting insurers' requirements.
  • A NYSDMV driver's abstract is required.
  • Will perform other duties as assigned by the Transportation Team Leader or Transportation Manager.
Requirements:
  • High School Graduate or GED
  • Good eye sight to ensure safe navigation of fleet vehicles
  • Ability to stand, bend, drive, and walk most of the day
