Job Description
The Licensed Practical Nurse (LPN/LVN) will provide skilled nursing care to patients in their homes, under the supervision of a Registered Nurse. Key responsibilities include:
- Developing and implementing individualized care plans that meet the unique needs of each patient.
- Delivering high-quality patient care in accordance with physician orders and Aveanna's policies and procedures.
- Collaborating with interdisciplinary teams to ensure comprehensive care and optimal patient outcomes.
- Communicating effectively with patients, families, and healthcare professionals to ensure seamless transitions and continuity of care.
Qualifications
To be successful in this role, you must:
- Hold a current, unrestricted state license as a Licensed Nurse in the state of practice.
- Have at least six months of recent experience as a Licensed Nurse in a clinical care setting.
- Demonstrate proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
