About the Role

We are seeking an experienced and skilled Optometrist to join our team at Sam's Club. As an Independent Optometrist, you will have the autonomy to establish your own practice and provide high-quality eye care services to our members.

Key Responsibilities
  • Provide comprehensive eye exams and vision care services to patients
  • Establish your own schedule and hours of operation
  • Control your patient schedule and set your own fee schedule
  • Keep 100% of exam/professional fees
  • Have the option to opt into equipment/instrumentation and administrative support services (if state allows)
Requirements
  • Doctor of Optometry (OD) from an accredited optometry school
  • Valid state optometry license
  • Ophthalmologist (MD or DO) from an accredited school (optional)
  • Valid state ophthalmology license (optional)
  • General Liability Certificate
  • Professional Liability Insurance Certificate
  • W-9
