About the Role

We are seeking a dedicated Speech Therapist to join our team at St. David's Healthcare at Home. As a member of our healthcare team, you will provide high-quality, patient-centered care by developing and conducting individualized therapy programs for patients with communication, cognitive, and/or swallowing impairments.

Key Responsibilities
  • Conduct comprehensive patient assessments and establish appropriate treatment goals.
  • Develop and implement individualized therapy programs to improve communication abilities.
  • Evaluate patient progress regularly and adjust programs as needed to maximize patient progress.
  • Document progress in patient charts and complete notes, charges, discharge notes, and patient communication.
  • Consult with Program Director and Rehab Physician/Medical Director regarding pre-screen assessments and physician consultations to determine appropriate placement of potential patients.
Requirements
  • Masters Degree in Speech Language Pathology from an accredited program.
  • License to practice speech language pathology within the state.
  • Certificate of Clinical Competency or eligibility for Clinical Fellowship Year.
  • Ability to work varying hours, including weekends and holidays.
  • Reliable transportation and proof of valid automobile liability insurance.
  • Must have valid drivers license.
