Job Summary

CVS Health is seeking a highly skilled Registered Nurse to join our team as a Case Manager RN in Kalamazoo, MI. As a Case Manager RN, you will play a critical role in assessing, planning, implementing, and coordinating case management activities to evaluate the medical needs of our members and facilitate their overall wellness.

Key Responsibilities
  • Develop proactive plans of care to address identified issues and enhance short and long-term outcomes.
  • Conduct thorough assessments of members' needs and benefits using clinical tools and information/data review.
  • Apply clinical judgment to incorporate strategies designed to reduce risk factors and barriers and address complex health and social indicators that impact care planning.
  • Collaborate with supervisors and other key stakeholders in the member's healthcare to overcome barriers in meeting goals and objectives.
  • Utilize case management processes in compliance with regulatory and company policies and procedures.
Requirements
  • Registered Nurse with active Michigan state license in good standing.
  • Minimum 3+ years clinical practical experience as an RN required.
  • Minimum 2+ years in one or more of the following areas: Care Management, discharge planning, LTC (long term care), or home health care coordination experience required.
  • Ability to travel 25-50% within Kalamazoo and surrounding counties.
Preferred Qualifications
  • Confidence working at home/independent thinker, using tools to collaborate and connect with teams virtually.
  • Certified Case Manager is preferred.
  • Excellent analytical and problem-solving skills.
  • Effective communications, organizational, and interpersonal skills.
  • Ability to work independently.
  • Effective computer skills including navigating multiple systems and keyboarding.
