Suncoast Center, Inc. is seeking a skilled Licensed Practical Nurse (LPN) to join our team in a community mental health setting. As a key member of our FACT Team, you will play a vital role in providing high-quality care to individuals diagnosed with severe and persistent mental illness.
Key Responsibilities- Collaborate with the team psychiatrist to coordinate medical assessments, schedule appointments, and administer medication as needed.
- Under the direction of the team psychiatrist, manage the system of getting medication to clients, ensuring accurate and up-to-date records.
- Record medication prescriptions, write orders, and ensure compliance with medication delivery.
- Order medications from the pharmacy, arrange for delivery or pickup, and ensure clients' medications are available and packaged correctly.
- Conduct nursing assessments, develop treatment plans, and collaborate with the case coordinator and other treatment team members to ensure medication administration and education are integrated into the plan.
- Use solution-oriented strategies and principles of recovery to provide direct clinical services, focusing on medication administration, education, and treatment plan goals.
- As case coordinator, ensure all continuing documentation is completed in a timely manner, including status letters, outcomes, FARS, treatment plan addendums, and reviews.
- Complete all other documentation correctly and in the required time frame, including progress notes with clear objectives and other documentation as requested.
- Participate in the FACT Team on-call response system, providing crisis intervention and on-call services as needed.
- Ensure all assessments, treatment plans, and progress notes are completed with accuracy, attention to detail, and demonstrate excellent clinical skills and judgment.
- Assist clients in developing realistic, effective, and measurable treatment plan goals and objectives, establishing and prioritizing behaviorally descriptive desired changes.
- Participate in daily staff organizational meetings and treatment planning review meetings, discussing cases appropriately and reviewing client contacts.
- Coordinate with client's family members, other FACT staff, and community supports to ensure client needs are met and all involved in the client's treatment work together.
- The ability to function and operate as a team player, showing dignity and respect for all.
- The ability to comply with all equal employment opportunity and ethical standards and regulations.
- The ability to take direction in a respectful and productive manner from supervisors and managers.
- The ability to work, as scheduled, in a timely fashion, and with an attitude oriented to service.
