Job Summary:

The LPN will be responsible for supervising nursing personnel to deliver nursing care and coordinating care delivery to ensure that residents' needs are met in accordance with professional standards of practice. The ideal candidate will have 2-3 years of clinical experience, with a focus on gerontological nursing, including dementia populations.

Responsibilities:
  • Supervise nursing personnel to deliver nursing care
  • Coordinate care delivery to ensure residents' needs are met
  • Develop and implement care plans in collaboration with the healthcare team
  • Monitor and report changes in residents' conditions
  • Provide education and training to caregivers
Requirements:
  • Valid and current LPN/LVN state license
  • 2-3 years of clinical experience
  • Gerontological nursing experience, including dementia populations
  • Ability to read, write, and speak English in an understandable manner
