We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Wellpath. As a LPN, you will play a vital role in providing high-quality patient care and making a positive impact in the lives of our patients.
Key Responsibilities:- Assess patients' physical, psychological, and social dimensions to develop individualized treatment plans.
- Implement treatment programs directed by healthcare practitioners, administering medications and performing diagnostic tests as needed.
- Collaborate with healthcare teams to provide comprehensive care and respond to health emergencies.
- Document nursing encounters using SOAP charting and communicate effectively with nursing staff, physicians, and other relevant personnel.
- Graduate from an accredited school of nursing.
- Prefer a minimum of one (1) year clinic experience.
- Have and maintain current licensure as a Licensed Practical/Licensed Vocational Nurse within the state of employment.
- Must be able to obtain and maintain CPR certification.
