POSITION SUMMARY:
The Youth Empowerment Specialist Case Manager is responsible for ensuring that assigned clients, specifically youth and young adults aged 14 to 24, receive a comprehensive array of client-focused services aimed at facilitating their success in employment and educational programs.
Our Learn to Earn initiative provides free educational and training opportunities, alongside Work Readiness and Job Placement support, for the youth of Rhode Island. Participants, whether currently enrolled in school or not, are paired with a dedicated case manager who will guide them in achieving their educational and employment aspirations.
WORK SCHEDULE REQUIREMENTS:
- This is a full-time position requiring 35 hours per week, with potential evening hours as needed. Flexibility in scheduling is essential to meet the needs of the client population, which may involve travel across various youth centers.
- Work may be conducted at different locations within the organization.
ESSENTIAL QUALIFICATIONS:
- Preferred candidates will possess an associate's or bachelor's degree, along with a minimum of two years of relevant experience or equivalent life experience.
- Demonstrated expertise in case management and knowledge of youth development is required.
- Experience working with urban youth and familiarity with fields such as human relations, psychology, or social work is essential.
- Strong interpersonal, written, and verbal communication skills are necessary, alongside excellent organizational abilities.
- Proficiency in Microsoft and Google applications is required.
- A valid driver's license and proof of vehicle insurance are mandatory.
- All necessary immunizations and vaccinations must be maintained.
CONFIDENTIALITY:
Access to program case management records is granted, and confidentiality must be upheld in accordance with applicable laws and organizational policies.
KEY RESPONSIBILITIES:
To excel in this role, the individual must effectively perform the following duties:
- Provide case management services to youth and young adults aged 14 to 24 enrolled in employment and educational initiatives.
- Conduct thorough intake assessments for clients.
- Monitor client cases by tracking attendance, evaluating service strategies, advocating for necessary resources, and providing crisis intervention and personal support.
- Develop and implement individualized service plans in alignment with grant requirements.
- Maintain accurate case records, complete necessary documentation, and generate monthly reports on attendance and case notes.
- Respond to inquiries from potential program partners and incoming youth.
- Devise strategies to retain youth through motivation and encouragement.
- Engage in outreach and networking with other youth-serving organizations and educational institutions.
WORK ENVIRONMENT:
