Work Schedule:
Monday - Friday
12:30 PM - 9:00 PM
Rotating weekends: Saturday and Sunday 8:30 AM - 7:00 PM
Compensation:
Shift Differential: $6.00 per hour
Sign-On Bonus: $10,000 payable after the initial 90 days of employment

Key Responsibilities:
- Review and assess physician medication orders for appropriateness, including drug selection, dosage, potential interactions, and administration routes. Provide recommendations for therapy adjustments to physicians and facility staff as necessary.
- Maintain a comprehensive patient database, ensuring accurate current and historical medical information, relevant clinical and laboratory data, and concurrent medication records.
- Ensure that necessary laboratory tests for monitoring high-risk conditions are ordered, evaluated, and discussed with the healthcare team.
- Collaborate with the healthcare team to provide clinical and technical support as required.
- Document all communications with physicians, nurses, and other healthcare team members, as well as patients and their families, in the medical record.
- Implement policies and procedures for the safe procurement, storage, distribution, and disposal of medications and biologicals.
- Maintain all required documentation in compliance with federal and state regulations.
- Notify the Pharmacy Manager of any medication-related errors.
- Assist facility staff with procedures related to the safe and effective management of medications.
- Perform additional tasks as assigned.
Qualifications:
- Bachelor's Degree in Pharmacy; Pharm D preferred.
- Minimum one year of experience in long-term care.
- Licenses/Certifications as required by regulatory agencies and business needs.
- Detail-oriented, goal-driven, team player with strong oral communication skills.
