Job Summary

Evergreen Family Medicine, PC is seeking a highly skilled and dedicated individual to fill the role of School Based Health Care Coordinator. This position will serve as a liaison between the school district and our clinic, providing access to healthcare for students in the Douglas County area.

Key Responsibilities
  • Collaborate with school administrators and our healthcare team to develop and implement school-based health programs.
  • Act as a patient advocate, ensuring that students and their families receive high-quality, integrated healthcare.
  • Provide support for quality control of school-based health equipment and oversee program fluidity within schools.
  • Work with our immunization staff to organize vaccination events and provide guidance to school faculty on student medical inquiries.
  • Stay up-to-date on pediatric and vaccine requirements, as well as state and federal regulations.
  • Maintain continuity among clinics, departments, and local work teams by documenting and communicating actions, irregularities, and continuing needs.
  • Assist management teams at sites with pediatric and immunization training needs, and promote our school-based health programs through advertising and outreach efforts.
  • Obtain medical consent from parents and maintain good communication throughout the process.
  • Provide education to students and parents on medication administration, as prescribed by our providers.
Requirements and Qualifications
  • Certified Clinical Medical Assistant or higher.
  • Excellent organizational and interpersonal skills.
  • Strong oral and written communication skills.
  • Effective time management skills and ability to work independently and as part of a team.
  • Proficiency with Microsoft Excel and consolidation software systems.
  • Valid driver's license and ability to work in a fast-paced environment.
