Position Overview:
Hamaspik HomeCare is in search of a dedicated full-time Registered Nurse to enhance our exceptional team. This role is essential in delivering top-notch and empathetic healthcare services to our clients and their families.

Key Responsibilities:
  • Perform initial evaluations of new clients to identify their healthcare requirements and create tailored care plans.
  • Conduct regular home visits to monitor client progress and modify care plans as needed.
  • Work collaboratively with the interdisciplinary team to guarantee optimal patient care.
  • Offer education and emotional support to clients and their families.
  • Keep precise and current patient records and documentation.
  • Deliver in-service training and guidance to Home Health Aides and Personal Care Assistants to ensure the highest standards of care.
  • Engage in team meetings and assist in the formulation of care strategies.
Qualifications:
  • Valid New York State RN license.
  • Minimum of one year of nursing experience, ideally in home care or a related field.
  • Strong communication and interpersonal skills.
  • Capability to work independently as well as part of a team.
  • Compassion and commitment to delivering patient-centered care.
  • Reliable transportation and a valid driver's license.
