The Jewish Family & Children's Service is seeking an experienced Aging Life Care Manager to join our team. As an Aging Life Care Manager, you will be responsible for evaluating the strengths and needs of older adults and adults living with chronic illnesses, educating clients and their families/care partners about options and resources, creating and managing client-centered plans of care, and facilitating client and family/care team meetings and relationships.
Key Responsibilities- Evaluate the strengths and needs of older adults and adults living with chronic illnesses
- Develop and implement client-centered plans of care
- Facilitate client and family/care team meetings and relationships
- Build relationships with community service providers and elder-care professionals
- Document care plans and care management
- Manage and advocate for appropriate and ongoing medical care
- Meaningful prior experience working with older adults in a clinical setting
- Proficiency with Microsoft Office Suite, especially Word and Outlook
- Excellent teamwork, communication, listening, leadership, and instruction skills
- A valid driver's license and a reliable, insured vehicle
