Family Support Specialist

About the Role
We are seeking a compassionate and dedicated Family Support Provider to join our team at the Community Counseling Center (CCC). As a Family Support Provider, you will play a vital role in empowering families to navigate the mental health system and achieve their goals.
Job Summary
The successful candidate will provide support and guidance to families with children who have been diagnosed with behavioral or emotional disorders. Your primary responsibility will be to assist families in accessing resources and services, while also advocating for their rights and interests.
Key Responsibilities
- Empowerment Through Knowledge: Provide parents and caregivers with essential information about their child/youth's needs, treatment options, and how to make informed decisions.
- Active Participation: Guide parents/caregivers through the planning process, emphasizing the importance of their voice in developing and implementing individualized treatment plans.
- Voice Amplification: Empower parents/caregivers to advocate effectively during planning meetings, ensuring their perspectives are heard and valued.
- Strengths-Based Approach: Work closely with families to highlight the significance of individualized planning, leveraging their inherent strengths to achieve positive outcomes.
- Team Collaboration: Educate families on the roles of various providers, promoting a cohesive 'team' approach to holistic care.
- Natural Supports: Discuss the benefits of natural family and community supports, encouraging utilization of existing networks.
- Problem-Solving Skills: Introduce effective problem-solving methods, developing strategies to address challenges.
- Transition Support: Assist parents/caregivers of Transition Age Youth, facilitating the shift from decision-maker to supportive role as youth become more independent.
- Community Connection: Link families to vital community resources, strengthening community bonds.
- Engagement and Re-Engagement: Foster active engagement with families, reconnecting with those who may have disengaged.
- Education Advocacy: Provide education on services available through CCC.
- Community Outreach: Engage with the community to raise awareness, educating about available resources.
- Clinical Team Participation: Collaborate as a valued member of the clinical team, contributing insights from the client's perspective.
- Documentation Excellence: Adhere to clinical policies, DMH guidelines, CARF standards, and federal/state program requirements.
- Client Transportation: Responsible for safe transportation of clients using agency or personal vehicles as needed.
