Job Description
The RN Case Manager will be responsible for assessing patient needs and developing personalized care plans to ensure timely throughput, safe discharge, and prevent avoidable readmissions. This involves reviewing patient activities related to medical necessity of hospitalization, appropriateness of length of stay, and utilization of services rendered.

This position requires knowledge and skills necessary to provide care based on physical, psychological, educational, safety, and related criteria for various age groups, including neonates, infants, children, adolescents, adults, and geriatrics. The ideal candidate will have excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.

Requirements
To be successful in this role, you will need:
- Associate Degree in Nursing, required
- Bachelor's Degree in Nursing, preferred
- Case Management certification, preferred
- Current Texas licensure with State Board of Nurse Examiners as an RN
- 1+ years experience in healthcare

Estimated Salary
$65,000 - $85,000 per year, depending on experience.
